MnCHOICES Assessments
MnCHOICES Assessment:services help senior’s make decisions about long-term care with the goal of people remaining in the community. Services include:
- Early intervention visits
- Pre-admission screenings prior to moving into a nursing home
- Information about public and private programs that can pay for services
- Transition assistance to help people move out of nursing homes
- Access community-based services.
MN DHS MnCHOICES Initiative Information
Care Coordination / Case Management
Care Coordination / Case Management services are provided to certain public program recipients. Includes assisting in locating and gaining access to:
- Medical
- Social
- Educational
- Other needed services, regardless of the funding source.
- Coordinating the plan of care, monitoring services to maximize health and safety, and eligibility reassessments.
